It may save you time at your first visit if you print out and complete the following forms and bring them with you. Otherwise, copies will be provided at your first visit.

Clients- Washington State requires all counselors and hypnotherapists to provide you with a copy of this document. You will need Adobe Acrobat Reader to download.
